Prepare for the day by completing one of the following application forms:

Be sure to read the Passport Book Application Checklist before Nov. 18 so you’ll be ready.  If you need to, you can also have your picture taken for a fee of $10, cash or money order.

The checklist will have all the fees and required documents. Hard copies of the passport application and ISIC forms can be picked up at the Center for International Service, Building 2A, Room 206. You can also access the forms from the Center’s Website.

Details on the requirements to obtain a passport in general and forms are online.

If you have any questions, contact the Center for International Service at intstudy@csi.cuny.edu or 718.982.2100.

Avoid the long lines at the Post Office and New York Passport Agency. Don’t let this convenient opportunity pass you by.
