The Office of Technology Systems (OTS) would like to implement a new process that will streamline move requests. It is often the case that OTS receives last-minute notification resulting in individuals not having equipment to perform their daily activities as well as inaccuracies in our personnel directory. In order to address these challenges, OTS, Campus Planning, and Human Resources collaboratively developed the CSI Move Request Form, which we would like to implement immediately. For future reference, the form is located on the OTS Help, Support, and Training Web page under “Helpful Forms.”
The Move Request Form is self-explanatory and upon submission will be routed automatically to all departments that need to be notified of the changes. It is important that this new procedure be followed.
