Each year, the Student Government recognizes those students, faculty, and staff members who have shown outstanding commitment to the College of Staten Island Community through their dedication and service with the College of Staten Island Student Government 2016 Student Service Awards.

Two awards are available in each of the following categories:
- Outstanding Graduating Senior
- Outstanding Administrator, Faculty or Staff Member
- Outstanding Non-Graduating Senior
One award is available in each of the following categories:
- Outstanding Freshman
- Outstanding Sophomore
- Outstanding Junior
- Outstanding Graduate Student
In order to be considered for the College of Staten Island Student Government 2016 Student Service Awards, each nominee must meet the following criteria:
1) Students must have a minimum 2.5 GPA
2) All nominees must have shown exemplary extracurricular service to the College of Staten Island community.
3) For student awards: You must attach a minimum of one letter of recommendation from a member of the CSI administration, faculty, or staff. You may also include letter(s) of recommendation from student(s) but at least one from administration, faculty, or staff.
4) For faculty or staff awards: You must attach a minimum of one letter of recommendation from a CSI student.
5) Letter(s) of recommendation should outline why the person is being nominated and include a list of the nominee’s service to the College of Staten Island community and the qualities that make this person a deserving recipient of this award.
6) Letter(s) should be no less than 150 words.
All nominees will be judged on their merit as outlined in their nomination packet.

DEADLINE: This form must be completed and submitted to the Student Government Office in the Campus Center (Building 1C), Room 207, by 4:30pm on Friday, Apr. 8, 2016.
