The State of New York will be mailing the 2015 Form W-2 (Wage and Tax Statement) directly to employees’ home addresses on record in the State Payroll System. This is the address on your paycheck. If this address is not correct, email the correct address to either dawn.derbyshire@csi.cuny.edu or susan.hannafin@csi.cuny.edu.
This request must be emailed by Monday, Nov. 16 in order for your W-2 to be sent to the correct address. W-2s that cannot be delivered will be returned to CSI Payroll at 395 Hudson Street. The W-2s will be postmarked no later than Sunday, Jan. 31, 2016.
